Screening Form Library

Questionnaires available to your practice. A published form is offered automatically wherever screening forms are used: the Screening Forms Smart Tag in Nursing Workflow and Provider Encounter templates, the Add Screening Form dialog, and Patient Intake.

You can view the screening form library but not change it.

Creating, editing and publishing questionnaires requires the Screening Form Manager permission.

Questionnaires supplied and maintained by Medgre. A practice can activate or deactivate them, but the questions and scoring rules cannot be modified.
